Position Summary: -
The Licensed Practical Nurse for home health is responsible for providing skilled nursing care to patients in accordance with physician's orders, directed by the registered nurse.

  Position Duties:
  
  • 
    
  • Provide nursing care and prescribed treatments to patients in accordance with physician's orders
    
  • Implement the patient's individualized plan of care, under the direction of the registered nurse
    
  • Assure the safety of the treatment procedure, patient and environment at all times

    • Possess effective communication and active listening skills
      
    • Current automobile insurance according to company policy
      
    • Must be able to speak and write in English
      
      Licensing/Certification/Education/Experience Requirements: - -
      
      • 
        
      • Graduate of an accredited practical nursing program
        
      • One (1) year experience as a licensed practical nurse preferred
        
      • One (1) year home health experience preferred
        
      • Current licensed practical nurse
        
      • Current CPR certification
        
      • Unrestricted driver's license
        
        Physical Requirements:
        
        • 
          
        • Ability to lift to 40 pounds
          
        • Work requires walking, standing, bending, reaching, kneeling, pushing and pulling
