*Overview*We are seeking a compassionate and dedicated *Licensed Practical Nurse (LPN)* to join our team in the *Long-Term Care (LTC) Rehabilitation* department. As an LPN in our facility, you will play a vital role in providing high-quality nursing care to residents who are undergoing rehabilitation or require long-term care.*Compensation:** *Local LPN*: $33.00 - $38.00 per hour*Responsibilities** Provide direct patient care, including administering medications, monitoring vital signs, and assisting with daily activities.* Support rehabilitation efforts by assisting patients with mobility exercises, physical therapy, and other recovery activities.* Collaborate with a multidisciplinary team, including physical therapists, occupational therapists, and other healthcare professionals, to create and implement individualized care plans.* Monitor and document patient progress, including any changes in condition, medications, and treatments.* Educate patients and families on care plans, medications, and lifestyle adjustments to promote recovery and well-being.* Supervise and guide nursing assistants and other support staff to ensure consistent, high-quality care.* Advocate for patients needs, ensuring that their concerns are addressed and care plans are followed.*Requirements** Valid Licensed Practical Nurse (LPN) license in the state of practice.* Current BLS certification* Valid drivers license* COVID-19 vaccination (or valid exemption)Join our team and make a meaningful impact on the lives of our patients while advancing your nursing career!Job Type: ContractPay: $33.00 - $38.00 per hourPhysical Setting:* Long term care* Nursing home* Rehabilitation centerWork Location: In personby Jobble
