Job Description Director of Case Management (RN, RCP, or SW) — Madison, WI HealthCare Support is seeking an experienced RN or Respiratory Therapist or Social Worker to join a mission-driven team as a Director of Case Management in the Madison, WI Area. This full-time, salaried role offers you the opportunity to over see staff, work with multiple different departments and help make a direct impact on patients experience. Coordinate patient care and transitions by overseeing case plans, facilitating interdisciplinary communication, and driving process improvements for optimal outcomes. Qualifications License as a RN, Respiratory Therapist or Master's in Social Work in the state of WI Minimum three (3) years of medical case management or similar experience Must have hospital case management experience 2 years of leadership experience within case management Interest ed ?
